Abstract
Endoscopic retrograde cholangiopancreatography (ERCP) has evolved from a diagnostic modality to a primarily therapeutic procedure for pancreatic and biliary disorders. However, ERCP alone or in association with therapeutic procedures can cause a variety of complications including pancreatitis, hemorrhage, perforation, and other conditions. Acute pancreatitis is the most common such complication. Currently, little is known regarding the pathophysiology of post-ERCP pancreatitis. Several factors, including chemical, mechanical, and microbiological factors are mentioned, and several cases have involved fracture of the floppy tip from the guidewire shaft and subsequent injury. Here, we report a case of severe acute pancreatitis as a result of hydrophilic guidewire tip fracture. (Korean J Med 75:S709-S714, 2008)
