Trans Abstract

Gastroesophageal reflux disease (GERD) is prevalent in Korea. Clinical manifestations include typical symptoms which are heartburn and regurgitation and atypical symptoms that can be esophageal or extraesophageal. Various extraesophageal manifestations are associated with GERD, and the most significant of these are chronic cough, laryngitis, and asthma. The diagnosis of extraesophageal syndrome is often difficult due to the poor sensitivity and specificity of available diagnostic modalities, including endoscopy, pH monitoring, and laryngoscopy. Consequently, empirical treatment with a proton pump inhibitor (PPI) is considered to be the most cost-effective tool for the diagnosis and management of patients suspected of having extraesophageal syndrome without alarm features. Those who do not respond to PPI therapy should undergo an additional work-up that includes monitoring esophageal pH or impedance, and motility tests to evaluate the relationship between reflux and symptoms. Surgical management should cautiously be considered for very carefully selected patients.

서 론

위식도역류 질환(gastroesophageal reflux disease)은 우리나라를 비롯한 아시아 국가에서도 식생활의 서구화, 고지방, 고열량 식이섭취의 증가, 비만 인구의 증가 등으로 점차 증가하는 경향이다. 국내 조사에 의하면 아산시 거주자를 대상으로 하였을 때 증상을 기준으로 3.5%의 유병률을 보고한 연구가 있었고 건강검진 수검자에서 내시경 이상 유무를 기준으로 미란성 식도염(erosive esophagitis)은 8%, 비미란성 역류 질환(nonerosive reflux disease)은 4%의 유병률을 보고한 바 있다[1,2].

위식도역류 질환은 위 내용물이 식도로 역류하여 불편한 증상을 유발하거나 이로 인하여 합병증을 유발하는 질환으로 정의된다[3,4]. 불편한 증상이란 ‘위식도역류와 연관된 증상’이 삶의 질에 나쁜 영향을 줄 때로 정의되며, ‘위식도역류와 연관된 증상’에는 전형적인 증상인 가슴쓰림(heartburn)과 신물이 올라오는 산역류(regurgitation)가 있고, 비전형적인 증상으로 비심인성 흉통(noncardiac chest pain), 연하곤란, 연하통 등의 식도 증상과 만성 기침, 쉰 목소리, 천식, 목 이물감 등의 식도 외 증상이 있다(Fig. 1) [4].

위식도역류 질환의 비전형적인 식도 외 증상은 다양한 발생 원인과 진단의 어려움 때문에 위식도역류 질환과의 뚜렷한 연관성을 밝히기가 어려우며 실제 과잉진단(overdiagnossis)이 되어온 측면도 있다[5]. 따라서 대부분의 가이드라인에서는 전형적인 식도 증상인 가슴쓰림이나 산역류가 함께 동반되거나 임피던스 산도 검사 등의 검사에서 역류 질환의 객관적인 증거가 있는 경우에만 위식도역류 질환에 준하는 치료를 권고하고 있다[3,5,6].

본고에서는 아직 명확하지 않은 부분이 많은 비전형적 식도 외 증상에 대해서 몬트리올 합의에서 위식도역류와 비교적 연관성이 많은 것으로 제시된 증후군을 중심으로 지금까지 밝혀진 지식에 대해 정리하고 진단 및 치료 방법에 대해서 살펴보고자 한다[4].

역류성 기침증후군(reflux cough syndrome)

기침은 3주 이내에 호전되는 급성 기침과 3-8주간 지속되는 아급성 기침, 8주 이상 지속되는 만성 기침으로 나누어 볼 수 있다. 만성 기침은 임상에서 흔히 접하게 되는 증상으로 안지오텐신전환효소(angiotensin converting enzyme) 억제제를 복용하지 않고, 정상 흉부 X-ray 소견을 가진 비흡연자에서 만성 기침의 가장 흔한 원인은 후비루(post-nasal drip syndrome), 천식, 위식도역류, 만성 기관지염 순으로 알려져 있으며, 이들 네 가지 원인이 대부분(> 90%)을 차지하고 있다[7,8].

위식도역류에 의한 만성 기침의 발생 기전은 2가지로 생각하고 있으며, 직접적인 위십이지장 내용물의 인후두로의 역류 및 기관지로의 흡인과 위식도역류에 의해 하부 식도의 미주신경이 자극되어 식도-기관지 반사(esophageal-bronchial neural cough reflex)가 유발되는 간접적인 자극이 있다.

만성 기침 환자가 내원하면 병력청취 및 신체 검사를 통해서 감염, 안지오텐신전환효소 억제제 사용 여부를 확인하고 흉부 X-ray를 시행하여 이상이 없는지 확인한 후 가장 흔한 원인인 후비루에 대해서 경험적으로 1세대 항히스타민제-비충혈제거제(antihistamine-decongestant combination)를 사용해 볼 수 있다. 2-3주간의 치료에 반응이 없으면 폐기능 검사를 통한 천식에 대한 평가가 이루어져야 하고 폐기능 검사에서 천식의 증거가 없다면 다음으로 흔한 원인인 위식도역류에 의한 만성 기침의 가능성을 고려해 볼 수 있다.

위식도역류에 의한 만성 기침 환자의 상당수가 전형적인 가슴쓰림과 역류 증상을 보이지 않고, 위식도역류를 기침의 원인으로 증명할만한 명확한 검사가 없어서 진단에 어려움이 있다[9]. 내시경 검사는 역류와 연관된 만성 기침 환자의 약 20% 정도에서만 이상 소견을 보이는 등 정상인 경우가 많아 민감도가 떨어진다[7]. 식도 산도 검사나 임피던스 산도 검사는 비정상적인 역류를 확인할 수 있으나 경험적 위산 억제 치료보다 침습적이고 인과관계에 대한 명확한 증거를 제시하는 데에 한계가 있다. 역류 검사상의 역류와 증상과의 연관성을 나타내기 위한 여러 가지 지표들인 증상 지수(symptom index, SI), 증상 민감 지수(symptom sensitivity index, SSI), 증상 관련 확률(symptom association probability, SAP)이 개발되어 있긴 하지만 지표 한 가지만 가지고 인과관계를 판단하기는 부족하며 증상 및 다른 검사 결과를 종합적으로 고려해야 하고 결과를 해석하는데 신중을 기할 필요가 있다[10].

이와 같이 민감하고 특이적인 진단 도구가 없는 상태에서, 진단 겸 치료 목적으로 경험적인 위식도역류 질환 치료를 하는 것이 현재까지는 비용-효과 면에서 가장 우월한 것으로 여겨지고 있다[9]. 한 연구에서는 위식도역류와 연관된 만성 기침으로 생각되는 환자를 대상으로 양성자펌프억제제(proton pump inhibitor)의 경험적 치료가 79%의 증상 호전을 보였다고 한다[11].

치료로는 생활습관 개선과 더불어 양성자펌프억제제 표준용량을 하루 2회 투여하게 되며, 반응이 있는 경우는 대체로 2주 안에 반응을 보인다[12]. 1-2개월 후에도 증상의 호전이 없으면 식도 내압 검사와 식도 산도 검사나 임피던스 산도 검사를 통해서 역류와 기침과의 연관성을 확인할 필요가 있다. 이러한 검사들을 통해 최적의 내과적 치료에도 반응하지 않는 비정상적 역류와 기침과의 관계가 객관적으로 규명된 경우에는 위저부주름술(fundoplication) 등의 수술적인 치료를 고려해 볼 수 있겠다.

치료로 증상이 호전된 경우에 얼마나 오래 치료를 해야 하는지는 알려진 바가 없으나 갑자기 치료를 중단하거나 약물을 줄이게 되면 재발하는 경우가 많으므로 서서히 약물을 줄이면서 증상의 악화 여부를 관찰해야 한다.

위식도역류를 포함한 다양한 기침의 원인에 대한 검사에 특이 소견이 없고 각종 치료에 반응이 없는 만성 기침의 경우 감각신경 장애로 인한 기침(sensory neuropathic cough)의 가능성을 생각해 볼 수 있다. 이는 미주신경(vagus nerve)이 자극에 대한 역치가 낮아져서 발생하는 것으로 생각되며, 치료도 대상포진후신경통(postherpetic neuralgia)과 같은 신경병성 통증과 비슷하게 삼환계 항우울제인 amitriptyline 10 mg/일 또는 알파2-델타 리간드인 pregabalin (150-300 mg/일)이나 gabapentin (100-900 mg/일)을 사용하며 일부 환자에서 좋은 치료 효과가 보고되었다[13-15].

역류성 후두염증후군(reflux laryngitis syndrome)

만성 후두염(chronic laryngitis)은 후두의 염증이 수주간 지속되는 현상으로 쉰 목소리(hoarseness), 인후통(throat pain), 인두구(globus pharyngeus), 잦은 목 청소(repetitive throat clearing), 성대 피로(voice fatigue), 기침, 연하곤란 등의 증상을 보인다. 대부분의 후두염은 담배연기 같은 자극성 물질에 의해 발생하지만 이러한 물질에 노출된 적이 없는 사람에서는 위식도역류가 주요한 원인이 되기도 한다. 이 경우를 인후두역류(laryngopharyngeal reflux)라고 한다. 후두에는 식도에서와 같은 산 청소 작용(esophageal acid clearance)이나 점막의 방어 작용이 없기 때문에 식도염에 비해 소량의 역류에도 인후두역류 질환이 발생할 수 있으며, 특히 산(acid) 이외에도 펩신(pepsin)에 의한 후두 상피세포의 손상이 발생할 수 있다[16,17].

위식도역류가 증상을 유발하는 기전은 다른 식도 외 증상과 유사하게 위내용물의 후두부 역류로 인한 직접적인 자극과 위내용물의 식도 역류에 의한 미주신경 자극을 매개로 한 간접적인 자극으로 설명을 한다.

후두염 역시 현재까지 명확한 진단 방법이 없고 적절한 진단법에 대해 논란이 많은 상태이다. 대부분 임상적인 증상과 후두경 검사(laryngoscopy) 소견을 참고로 하여 의심을 하게 되며 후두경상 후두 부종, 성대 부종, 가성대구(pseudosulcus), 후두 육아종 등의 소견을 보일 수 있으나 정상인에서도 흔히 발견되고 후두경 소견의 판단에 주관적인 측면이 많아 큰 도움이 되지는 않는다[18,19]. 또한 내시경 소견이나 식도 산도 검사도 별로 도움이 되지 못하며, 검사의 민감도를 높이기 위해 하부식도와 함께 상부식도의 pH를 동시에 측정하는 24시간 이중 탐침 산도 검사(24hour double-probe pH monitoring)가 개발되었으나 인후두역류 질환의 54%에서만 양성 소견을 보이고 정상인에서도 43%에서 양성 소견을 보여 아직 임상적 유용성이 확립되지 못했다[20-22]. 침(saliva) 안의 펩신에 대한 효소활성도를 측정해서 상부식도의 위산역류를 쉽게 확인해 보려는 노력도 있었다[23]. 그러나 인후두역류 진단을 위한 침 속 펩신 측정의 임상적 유용성에 대해 몇몇 연구들에서는 서로 상반된 결과를 보여 주었다[24-26]. 최근 펩신에 대한 항체를 이용하여 빠르고 쉽게 침 속의 펩신 유무를 확인할 수 있는 진단 키트 형태의 측방 유동(lateral flow) 분석 디바이스(PeptestTM, RD Biomed Ltd, Cottingham, UK)를 개발하여 위식도역류 질환의 진단에 있어서 임상적 유용성을 살펴본 연구가 있었으며, 만족할 만한 민감도(87%), 특이도(87%)와 양성예측도(87%), 음성예측도(78%)를 보여주었으나 인후두역류를 진단하기 위한 임상적 가치에 대해서는 추가적인 연구가 필요한 상태이다[27]. 결국 만성 기침과 마찬가지로 후두염 역시 표준 진단 방법이 없기 때문에 경험적인 위산분비억제 치료를 하게 되나, 과민성 장증후군(irritable bowel syndrome)에서처럼 위약(placebo) 효과가 약 40% 정도로 보고되기 때문에 치료 반응을 보고 진단을 하기에는 제한점이 있다.

인후두역류가 의심되면서 특별한 경고 증상이나 징후가 없는 경우에는 식사 조절과 생활습관 개선을 하면서 경험적으로 양성자펌프억제제를 표준용량 하루 1회 혹은 2회로 1-2달간 투여해 본다. 증상 호전이 있으면 후두 손상이 치유되도록 6개월간 지속적으로 투여한 후 점차 용량을 줄여서 최소한의 산 억제로 증상이 없도록 한다[28]. 치료에 반응이 없는 경우에는 치료를 유지하면서 임피던스 산도 검사나 식도 산도 검사를 통해 증상 발생의 원인이 역류인지 다른 원인이 있는지 감별하는 노력이 필요하다.

일반적으로 전형적인 위식도역류 질환의 증상이 있는 경우에는 양성자펌프억제제 치료가 도움이 될 가능성이 높으나 그렇지 않은 경우에는 위산억제 치료가 권장되지는 않는다.

역류성 천식증후군(reflux asthma syndrome)

위식도역류는 천식 환자에서 30-80%의 유병률로 흔하게 발견되며 또한 천식을 악화시키는 요인 중의 하나로 잘 알려져 있다[29-33]. 그러나 천식에 대한 위산억제 치료의 효과를 본 무작위 연구들에 의하면, 천식 환자에서 위식도역류가 흔하기는 하나 위식도역류 질환의 전형적인 증상이 없는 환자에서는 위식도역류가 천식을 일으키는 주된 요인은 아닌 것으로 생각된다[34,35].

위식도역류가 기관지 수축을 일으켜서 천식 환자의 증상을 악화시키는 기전으로 위 내용물의 기관지 내로의 흡인으로 인한 직접적인 자극과 이로 인한 기관지의 수축, 위 내용물이 식도 하부의 미주신경을 자극하여 반사적으로 기관지 수축을 유발하는 기전이 알려져 있다[36-39]. 태생학적으로 기관기관지 나무(tracheobronchial tree)와 식도가 공통적으로 앞창자(foregut)에서 유래하고 식도와 기관지 모두 미주신경을 통하여 자율신경계 자극을 받기 때문에 이러한 반사 작용이 나타나게 된다.

다른 식도 외 증상들과 마찬가지로 위식도역류에 의한 천식의 표준 진단 방법은 아직까지 정해지지 못하였다[40]. 위식도역류 질환과 연관된 천식도 다른 원인에 의한 천식과 마찬가지로 주로 천명(wheezing)과 숨가쁨(shortness of breath)을 호소한다[40]. 특히 위식도역류 질환의 전형적인 증상이 동반되어 있고, 야간에 증상이 자주 나타나고, 위식도역류와 천식 증상이 동시에 나타나는 경우에는 위식도역류에 대한 치료로 천식 증상의 호전을 기대해 볼 수 있다. 특별한 경고 증상이나 징후(체중 감소, 빈혈, 연하곤란, 연하통 등)가 없는 경우 표준용량의 양성자펌프억제제를 하루 2회 투여하고, 생활습관 개선을 하도록 하면서 반응을 지켜본다. 또한 전형적인 위식도역류 질환의 증상이 없더라도 천식이 잘 조절되지 않는 경우에는 위식도역류에 대한 검사를 시행해 볼 수 있고, 검사상 명확한 역류의 증거가 없더라도 경험적으로 양성자펌프억제제 치료를 해 볼 수 있다[41]. 2-3개월 동안의 경험적 치료에 천식의 호전을 보이는 경우에는 위식도역류 질환을 천식의 악화 요인으로 생각할 수 있다. 천식의 호전은 최대 호기 유속(peak expiratory flow rate)이 20% 이상 호전되거나 천식 증상이 호전되거나 경구 스테로이드 복용량이 20% 이상 줄어들 때로 정의되기도 한다[42]. 이와 같은 천식의 호전을 보이는 경우에는 양성자펌프억제제의 장기 투여를 하게 되며, 보통 장기 투여 시에는 양성자펌프억제제 표준용량을 하루 1회로 줄여 보기도 하나, 증상의 악화를 보이는 경우에는 다시 2회 투여로 늘려야 한다.

양성자펌프억제제 투여에도 천식의 호전이 없는 경우에는 투약을 지속하면서 식도 산도 검사 또는 임피던스 산도 검사를 시행해서 증상 지속의 원인이 위식도역류가 현재 약으로 적절히 조절되지 않아서인지, 또는 위식도역류가 현재 천식의 주된 악화 요인이 아닌 상황인지를 확인해 본다. 위식도역류가 적절히 조절되지 않는 경우 항역류 수술(antireflux surgery)을 고려해 볼 수 있으나 인후두역류와 마찬가지로 천식에 대한 항역류 수술 치료의 효과는 아직 명확하게 규명되어 있지 않다. 일부 연구에서 수술적 치료가 천식 증상의 완화를 가져오긴 하였지만 폐기능 자체의 호전은 보이지 않았다[43,44]. 따라서 객관적인 위식도역류가 증명된 환자만을 대상으로, 환자와 수술적 치료의 제한점에 대한 심도 깊은 면담을 한 후에 수술 여부를 결정해야 한다.

결 론

기침, 후두염, 천식이 모두 다양한 원인이 함께 작용하여 발생하게 되는 질환(multifactorial disease)이고 위식도역류 질환은 그중 하나의 악화 인자일 뿐이며 독자적인 원인으로 작용하는 경우는 매우 드물다. 그러므로 만성 기침이나 후두염, 천식의 증상으로 내원한 환자는 위식도역류 외에 다양한 원인에 대해서 주의 깊게 평가되어야 하고, 위식도역류는 일부 환자에서만 주된 악화 인자로 작용한다는 사실을 주지할 필요가 있다. 또한 현재까지의 연구 결과에 의하면 식도 외 증상은 전형적인 위식도역류 질환의 증상이 있거나 내시경 혹은 역류 검사에서 비정상적인 위식도역류의 객관적 증거가 있는 경우에만 치료 효과를 기대해 볼 수 있다.

위식도역류 질환의 식도 외 증상에 대한 진단은 여전히 불확실한 측면이 많고 쉽지 않은 것이 현실이다. 임상에서는 진단 및 치료를 위해 경험적인 산분비억제 치료(표준용량 양성자펌프억제제 하루 2회)를 2-3개월 시도해 보고, 최적의 양성자펌프억제제 치료에도 불구하고 식도 외 증상이 지속되는 환자는 다른 관련 과(이비인후과, 호흡기내과, 알레르기 내과)들과 협의하고 상황에 따라 식도 산도 검사나 임피던스 산도 검사를 추가하여 다른 원인을 찾아보는 노력이 필요하다. 적극적인 내과적 치료에도 불구하고 산역류가 지속되면서 전형적인 증상을 호소하는 경우, 큰 식도 열공 헤르니아가 함께 있는 경우, 과량의 위식도역류가 증상의 원인인 경우 등 일부 환자들에서는 수술적인 접근을 고려해 볼 수 있지만, 적극적인 양성자펌프억제제 치료에 반응이 없다고 수술적 치료를 고려해서는 안되며, 역류 외의 다른 원인에 의한 증상일 가능성이 높으므로 이에 대한 평가를 시행하여야 한다(Fig. 2) [45].

최근 우리나라에서도 위식도역류 질환에 대한 유병률이 증가하고 있으나 전형적인 증상 외에도 비전형적인 증상을 호소하거나 증상이 없는 경우도 많이 있기 때문에 이에 대한 주의와 관심을 가질 필요가 있다. 특히 비전형적인 식도 외 증상에 대해서는 향후 최적의 진단 및 치료에 대한 양질의 연구가 필요하다.
