<trans-abstract xml:lang="en"><p>The topic of the extragastric manifestations of <italic>Helicobacter pylori</italic> infection continues to capture the attention of many researchers all over the world. There are, in fact, several studies concerning obesity, diabetes mellitus, cardiovascular diseases, lung diseases, hematologic diseases, eye and skin diseases, hepatobiliary diseases, and  neurological disorders. Among them, obesity and related disorders are hot spots in modern society. Here, we review the results of the studies about the relationship between <italic>Helicobacter pylori</italic> infection and obesity related diseases. <title>서 론</title>
<p>헬리코박터 파이로리(<italic>Helicobacter pylori</italic>)는 나선모양을 띄는 그람 음성 간균으로 위 상피조직에 분포하며 만성 위염, 소화성 궤양, 그리고 위암을 일으킨다는 것은 익히 알려진 사실이다[<xref ref-type="bibr" rid="b1-kjm-84-6-781-3">1</xref>,<xref ref-type="bibr" rid="b2-kjm-84-6-781-3">2</xref>]. 위점막이 헬리코박터 파이로리에 감염되면 급성기에는 다형핵 백혈구의 침윤이 초래되고 감염이 만성화되면 면역 반응에 동반되어 침윤한 단핵구가 지속적으로 위점막의 염증을 유발한다. 만성 염증상태는 위선의 위축을 야기하여 위체부 및 저부에서 분비되는 호르몬의 분비량을 감소시키며 염증유발성 사이토카인을 생성한다. 그리고 이러한 사이토카인이 혈액을 통하여 전신에 영향을 미치는 것으로 추정하고 있다[<xref ref-type="bibr" rid="b3-kjm-84-6-781-3">3</xref>,<xref ref-type="bibr" rid="b4-kjm-84-6-781-3">4</xref>]. 이에 저자는 다양한 내분비 관련 질환 중 헬리코박터 파이로리 감염과 관련된다고 여겨지는 비만 및 이와 연관되는 당뇨병, 심혈관 질환 및 뇌혈관 질환 등에 대하여 다루어 보고자 한다.</p>
</sec>
<sec sec-type="others">
<title>본 론</title>
<sec><title><italic>헬리코박터 파이로리</italic> 감염과 비만</title>
<sec>
<title>내분비 기관으로서의 위와 그렐린</title>
<p>헬리코박터 파이로리 감염과 제거 그리고 체중의 변화에 대한 내용을 살펴보기 위해서는 우선 위의 내분비 기관으로서의 기능에 주목해야 하며 특히 그렐린(ghrelin)에 대하여 알아볼 필요가 있다. 뇌하수체, 갑상선, 부신 등 전통적인 내분비 기관뿐 아니라 대부분의 말초 기관들 역시 내분비 기능을 가지고 있는데 이 중 특히 위는 섭식행동과 에너지 균형에 관여하는 내분비 기관이라고 할 수 있다. 위에서 분비되는 물질인 그렐린은 섭식행동을 촉진하는 강력한 효과를 가지는 호르몬인데 주로 위저부에서 분비되고 성장호르몬의 분비를 촉진 시킨다[<xref ref-type="bibr" rid="b5-kjm-84-6-781-3">5</xref>]. 혈중 그렐린 수치는 상황에 따라 변하는데 단기간의 공복 시 혈중 그렐린은 현저히 상승하며 식사를 하고 나면 급격히 감소한다[<xref ref-type="bibr" rid="b6-kjm-84-6-781-3">6</xref>]. 비만인 경우 정상에 비하여 혈중 그렐린은 감소하는데 에너지 과잉상태에서 섭식을 제한하기 위한 기전이라고 생각한다[<xref ref-type="bibr" rid="b7-kjm-84-6-781-3">7</xref>]. 고용량의 그렐린을 외부에서 주입하였을 때 섭식행위가 촉진되고 성장호르몬이 증가하며 체질량지수가 증가하는 현상을 보이는데 이는 혈중 그렐린이 증가할 때 체중이 증가한다는 증거라고 할 수 있다[<xref ref-type="bibr" rid="b8-kjm-84-6-781-3">8</xref>]. 그렐린은 다양한 경로를 통하여 렙틴과 같은 다른 호르몬들과 상호 작용하면서 식욕중추에 영향을 미친다[<xref ref-type="bibr" rid="b9-kjm-84-6-781-3">9</xref>]. 전술한 바와 같이 그렐린의 주요 작용이 영양섭취에 관한 것이지만 그렐린은 적정 수준의 혈당을 유지하는 기능도 담당하는데 이것은 아마도 성장호르몬 분비를 자극하기 때문일 것으로 여겨진다[<xref ref-type="bibr" rid="b10-kjm-84-6-781-3">10</xref>]. 이처럼 위는 단순히 음식물을 분해하고 영양분을 섭취하는 기관만이 아니라 그렐린이라는 호르몬과 연계되어 섭식행동 자체에 영향을 주는 중요한 내분비 기관이라고 할 수 있다.</p> 
</sec>
<sec><title>헬리코박터 파이로리 감염과 그렐린</title>
<p>그렐린을 분비하는 세포들이 위에 분포하기 때문에 만성 위축성 위염이 진행됨에 따라 그렐린의 생성과 분비가 영향을 받고 이에 따른 변화가 섭식행동에 영향을 주어 체중의 변화에까지 이어질 수 있다. 헬리코박터 파이로리 감염군에서 위점막에서 생성되는 그렐린이 감소되어 있으며 혈중 그렐린 역시 동반되어 감소되어 있다. 여러 연구에서 그렐린 분비세포의 수는 헬리코박터 파이로리에 감염된 경우에 현저히 감소되어 있는 것으로 밝혀졌다[<xref ref-type="bibr" rid="b11-kjm-84-6-781-3">11</xref>-<xref ref-type="bibr" rid="b13-kjm-84-6-781-3">13</xref>]. 그리고 그렐린 분비세포의 수는 위축성 위염이 심할수록 더욱 감소되어 있었다[<xref ref-type="bibr" rid="b11-kjm-84-6-781-3">11</xref>,<xref ref-type="bibr" rid="b14-kjm-84-6-781-3">14</xref>]. 혈중 그렐린은 헬리코박터 파이로리 감염군에서 대조군에 비하여 현저히 낮아 위축에 따라 그렐린의 혈중농도가 낮아진다는 것을 알 수 있었다[<xref ref-type="bibr" rid="b15-kjm-84-6-781-3">15</xref>]. 혈중 그렐린은 위의 위축이 관찰될 때만 저하되어 있었으며 위축성위염의 진행 정도와 혈중 그렐린의 수치는 반비례 관계를 보여 주었는데[<xref ref-type="bibr" rid="b16-kjm-84-6-781-3">16</xref>,<xref ref-type="bibr" rid="b17-kjm-84-6-781-3">17</xref>] 위점막에서 그렐린의 분비세포의 수가 위염의 정도에 반비례하는 것과 일치하는 결과라고 할 수 있다.</p>
</sec> 
<sec><title>헬리코박터 파이로리 제균과 그렐린</title>
<p>Nwokolo 등[<xref ref-type="bibr" rid="b18-kjm-84-6-781-3">18</xref>]은 12명의 건강한 보균자를 대상으로 실행한 초기 연구에서는 헬리코박터 파이로리를 제거했을 때 혈중 그렐린 수치가 증가한다고 보고하였다. 그러나 다른 연구에서는 헬리코박터 파이로리를 제거했을 때 혈중 ghrelin 수치는 특별한 변화를 보이지 않았다[<xref ref-type="bibr" rid="b15-kjm-84-6-781-3">15</xref>,<xref ref-type="bibr" rid="b19-kjm-84-6-781-3">19</xref>]. 이후 동물실험 등을 포함한 다양한 연구가 시행되었으나 헬리코박터 파이로리 제균과 혈중 그렐린의 관계에 대해서는 적절한 결론을 내릴 수 없었다[<xref ref-type="bibr" rid="b20-kjm-84-6-781-3">20</xref>-<xref ref-type="bibr" rid="b22-kjm-84-6-781-3">22</xref>]. 헬리코박터 파이로리 제균 전 후, 위점막의 그렐린 수치 역시 연구되었는데, 헬리코박터 파이로리 제균 후 위점막의 그렐린 수치는 현저히 증가되었으며 그렐린 분비 세포의 수도 증가되었다[<xref ref-type="bibr" rid="b12-kjm-84-6-781-3">12</xref>,<xref ref-type="bibr" rid="b23-kjm-84-6-781-3">23</xref>,<xref ref-type="bibr" rid="b24-kjm-84-6-781-3">24</xref>]. 헬리코박터 파이로리 제균과 관련하여 혈중 그렐린과 위점막 그렐린 수치가 다르게 나타난 것에 대하여서는 아직 명확한 설명을 할 수 없는 상태이며 향후 연구자들이 풀어야 할 숙제라고 생각한다.</p> 
</sec>

<sec><title>헬리코박터 파이로리 감염과 비만 그리고 헬리코박터 파이로리 제균 치료의 효과</title>
<p>언급한 바와 같이 헬리코박터 파이로리 감염이 만성 위축성 위염을 유발하고 이에 따라 위점막에서 그렐린의 분비가 감소하고 혈중 그렐린이 감소하는 것까지는 비교적 명확히 밝혀졌다고 할 수 있으나 결국 이러한 기전이 비만으로 이어지는가에 대해서는 아직까지 논란 중이라고 할 수 있다. 일부 연구에서는 헬리코박터 파이로리 감염과 과체중은 서로 관계가 없으며 체질량지수와 CagA 양성 헬리코박터 파이로리 감염도 별 다른 관계가 없다고 하였으나[<xref ref-type="bibr" rid="b25-kjm-84-6-781-3">25</xref>,<xref ref-type="bibr" rid="b26-kjm-84-6-781-3">26</xref>] Wu등[<xref ref-type="bibr" rid="b27-kjm-84-6-781-3">27</xref>]은 청소년기 헬리코박터 파이로리 감염이 비만을 예방한다고 하였다. 반면에 Perdichizzi 등[<xref ref-type="bibr" rid="b28-kjm-84-6-781-3">28</xref>]은 헬리코박터 파이로리 감염이 있을 때 체질량지수가 증가하며 비만이 생길 위험도가 상승한다고 하였다. 또한 Danesh 등[<xref ref-type="bibr" rid="b29-kjm-84-6-781-3">29</xref>]이 10,000명을 대상으로 메타분석한 바에 의하면 헬리코박터 파이로리 감염군의 체질량지수가 헬리코박터 파이로리 비감염군에 비하여 높았으나 심혈관 질환 위험도에서는 차이가 없었다. 이렇듯이 아직까지 헬리코박터 파이로리 감염이 과연 과체중과 비만을 억제하는 역할을 하는지[<xref ref-type="bibr" rid="b12-kjm-84-6-781-3">12</xref>] 혹은 과체중과 비만을 유발하는 역할을 하는지 또는 어떠한 역할도 하지 않는지에 대해서는 명확히 밝혀진 바가 없는데 이것은 과체중 혹은 비만은 어떤 단일 요인으로 결정될 수 있는 것이 아니라 개인의 습관, 사회경제적 환경, 문화적 요소 등 다양한 요인에 영향을 받기 때문이라고 생각한다.</p> 
</sec>
<sec><title>헬리코박터 파이로리 제균과 체중변화</title>
<p>헬리코박터 파이로리 제거 후, 위 점막의 그렐린 분비가 증가하고 이에 따라 혈중 그렐린 수치가 높아져 체중이 증가한다는 것은 받아들일 만한 가설이라고 할 수 있다[<xref ref-type="bibr" rid="b12-kjm-84-6-781-3">12</xref>,<xref ref-type="bibr" rid="b18-kjm-84-6-781-3">18</xref>]. 몇몇 연구에서는 헬리코박터 파이로리를 제거하였을 때 몸무게가 증가한다는 보고가 있었으며[<xref ref-type="bibr" rid="b30-kjm-84-6-781-3">30</xref>-<xref ref-type="bibr" rid="b32-kjm-84-6-781-3">32</xref>] Lane 등[<xref ref-type="bibr" rid="b30-kjm-84-6-781-3">30</xref>]이 최근 발표한 바에 따르면 헬리코박터 파이로리 감염자 중 헬리코박터 파이로리를 제거한 군에서 제거를 하지 않은 군에 비하여 체질량지수가 현저히 증가하는 것을 관찰할 수 있었으며, 일본에서도 비슷한 결과가 발표되었다[<xref ref-type="bibr" rid="b31-kjm-84-6-781-3">31</xref>,<xref ref-type="bibr" rid="b32-kjm-84-6-781-3">32</xref>]. 그러나 헬리코박터 파이로리를 제거한 후 입맛이 좋아져서 식사량이 늘고 이에 따라 체질량지수가 증가했을 가능성이 있으며 제균 후 혈중 그렐린 농도가 증가한다는 명확한 결론이 없기 때문에 섣불리 헬리코박터 파이로리 제균이 체질량지수의 증가를 유발했다고 결론내릴 수는 없다.</p> 
</sec>
<sec><title>헬리코박터 파이로리 감염과 인슐린저항</title>
<p>인슐린저항은 복부비만 환자에서 심혈관 질환의 위험도와 제2형 당뇨병의 위험도가 높은 경우를 선별해내는 과정에서 성립된 개념이라고 할 수 있는데 인슐린저항과 그에 동반되는 고지혈증은 대사증후군의 중요한 유발인자라고 할 수 있다[<xref ref-type="bibr" rid="b33-kjm-84-6-781-3">33</xref>]. 전 세계적으로 비만 환자가 증가함에 따라서 인슐린 저항 및 이에 관계된 질환이 증가하였고 인슐린저항은 대사성증후군의 독립위험인자로 부상하게 되었다[<xref ref-type="bibr" rid="b34-kjm-84-6-781-3">34</xref>-<xref ref-type="bibr" rid="b36-kjm-84-6-781-3">36</xref>]. 인슐린 저항을 판단하는데 가장 널리 쓰이는 방법은 homeostatic model assessment-insulin resistance (HOMA-IR)인데[<xref ref-type="bibr" rid="b37-kjm-84-6-781-3">37</xref>,<xref ref-type="bibr" rid="b38-kjm-84-6-781-3">38</xref>] 공복 시 채혈만으로 다른 정교한 검사방법으로 얻어지는 결과와 비등한 결과를 얻을 수 있기 때문이다.</p> 

<p>이러한 HOMA-IR 분석을 통하여 인슐린저항과 헬리코박터 파이로리 감염의 관계를 조사한 9개의 논문[<xref ref-type="bibr" rid="b39-kjm-84-6-781-3">39</xref>-<xref ref-type="bibr" rid="b46-kjm-84-6-781-3">46</xref>]을 기반으로 분석한 자료에 의하면 헬리코박터 파이로리 감염과 HOMA-IR는 상관관계를 보여주어 결국 헬리코박터 파이로리 감염과 인슐린저항은 모종의 관계를 가지는 것으로 보인다[<xref ref-type="bibr" rid="b47-kjm-84-6-781-3">47</xref>].</p> 
</sec>
<sec><title>헬리코박터 파이로리 감염과 당뇨병</title>
<p>헬리코박터 파이로리 감염과 비만의 관계에 대한 논의와 마찬가지로 헬리코박터 파이로리 감염과 제2형 당뇨병의 관계에 대해서도 뜨거운 논쟁이 지속되어 왔으나 아직도 어떠한 결론을 내고 있지는 못하다. 몇몇 연구에 의하면 당뇨병 환자에서 헬리코박터 파이로리 감염의 감염률은 현저히 높다고 하였다[<xref ref-type="bibr" rid="b28-kjm-84-6-781-3">28</xref>,<xref ref-type="bibr" rid="b48-kjm-84-6-781-3">48</xref>-<xref ref-type="bibr" rid="b54-kjm-84-6-781-3">54</xref>]. 헬리코박터 파이로리 감염률은 당뇨병의 유병기간이 긴 경우[<xref ref-type="bibr" rid="b49-kjm-84-6-781-3">49</xref>,<xref ref-type="bibr" rid="b51-kjm-84-6-781-3">51</xref>], 소화불량증이 동반된 경우[<xref ref-type="bibr" rid="b54-kjm-84-6-781-3">54</xref>,<xref ref-type="bibr" rid="b55-kjm-84-6-781-3">55</xref>] 그리고 심혈관 자율신경병증이 있는 환자[<xref ref-type="bibr" rid="b54-kjm-84-6-781-3">54</xref>,<xref ref-type="bibr" rid="b56-kjm-84-6-781-3">56</xref>]에서 높게 나타났다. 또한 여성인인 경우, 체질량지수가 높은 경우, 중년인 경우, 혈압과 공복혈당 그리고 HbAIc가 잘 조절되지 않은 경우에도 헬리코박터 파이로리 감염률이 높았다고 하였다[<xref ref-type="bibr" rid="b57-kjm-84-6-781-3">57</xref>]. 이런 내용을 정리해 보면 헬리코박터 파이로리 감염은 유병기간이 길고 소화불량증세와 심혈관 자율신경계 이상을 동반하며 혈압과 공복혈당 그리고 HbAIc가 높은 비만한 중년 여성 당뇨병 환자에서 많이 관찰된다고 할 수 있다[<xref ref-type="bibr" rid="b28-kjm-84-6-781-3">28</xref>,<xref ref-type="bibr" rid="b48-kjm-84-6-781-3">48</xref>-<xref ref-type="bibr" rid="b58-kjm-84-6-781-3">58</xref>]. 이와 대조적으로, 다른 연구들은 헬리코박터 파이로리 감염률이 당뇨병환자와 대조군 사이에 그리고 특별한 차이점을 보이지 않는다고 하였으며 앞서 언급한 특정 상황에서도 차이가 없다고 하였다[<xref ref-type="bibr" rid="b59-kjm-84-6-781-3">59</xref>-<xref ref-type="bibr" rid="b64-kjm-84-6-781-3">64</xref>]. 미세혈관병증은 헬리코박터 파이로리의 감염의 음성 예측인자가 될 수 있다고 하였는데 미세혈관병증에 의한 위점막의 허혈성 변화가 헬리코박터 파이로리의 생존에 부정적인 영향을 미치기 때문으로 추측하였다[<xref ref-type="bibr" rid="b57-kjm-84-6-781-3">57</xref>]. 흥미로운 점은 헬리코박터 파이로리의 감염이 오히려 당뇨병 환자에서 더 적었다는 결과도 있다는 점이다[<xref ref-type="bibr" rid="b65-kjm-84-6-781-3">65</xref>,<xref ref-type="bibr" rid="b66-kjm-84-6-781-3">66</xref>]. 당뇨병성 합병증과 헬리코박터 파이로리 감염의 관계에 대한 연구는 아직까지 미미한 실정이며 역시 논쟁 중이라고 할 수 있다. 헬리코박터 파이로리의 감염과 제2형 당뇨병의 합병증에 대하여서는 단지 몇몇 연구가 있을 뿐인데 헬리코박터 파이로리의 감염과 신병증, 망막병증, 미세혈관병증은 밀접한 관계를 보이지 않았다고 보고한 연구[<xref ref-type="bibr" rid="b54-kjm-84-6-781-3">54</xref>,<xref ref-type="bibr" rid="b56-kjm-84-6-781-3">56</xref>]가 있는 반면 병독성이 강한 CagA 양성 헬리코박터 파이로리에 감염된 경우는 제2형 당뇨병 환자에서 대혈관병증[<xref ref-type="bibr" rid="b57-kjm-84-6-781-3">57</xref>], 신경병증[<xref ref-type="bibr" rid="b57-kjm-84-6-781-3">57</xref>], 미세알부민뇨 신장병증[<xref ref-type="bibr" rid="b67-kjm-84-6-781-3">67</xref>]이 동반되는 경우가 많았다고 분석하였는데 저자들은 헬리코박터 파이로리 감염에 동반된 전신 면역반응이 신장에서 알부민의 누출을 유발하기 때문일 것이라고 추정하였다[<xref ref-type="bibr" rid="b67-kjm-84-6-781-3">67</xref>]. 헬리코박터 파이로리 제균과 관련하여 당뇨병 환자에서 특이한 점은 제균율이 상당히 낮으며[<xref ref-type="bibr" rid="b62-kjm-84-6-781-3">62</xref>,<xref ref-type="bibr" rid="b68-kjm-84-6-781-3">68</xref>,<xref ref-type="bibr" rid="b69-kjm-84-6-781-3">69</xref>] 재감염률도 높다는 것이다[<xref ref-type="bibr" rid="b62-kjm-84-6-781-3">62</xref>,<xref ref-type="bibr" rid="b69-kjm-84-6-781-3">69</xref>,<xref ref-type="bibr" rid="b70-kjm-84-6-781-3">70</xref>]. 이러한 현상은 당뇨병 환자에서는 위의 미세혈관들이 손상을 입게 되어 항생제의 흡수가 감소하며 잦은 항생제 사용에 따라 헬리코박터 파이로리가 내성을 획득하기 때문이라고 생각된다[<xref ref-type="bibr" rid="b68-kjm-84-6-781-3">68</xref>,<xref ref-type="bibr" rid="b71-kjm-84-6-781-3">71</xref>]. 3제 요법에 실패한 경우 4제 요법을 사용하였을 경우에는 제균율이 확연히 상승하지만 이에 동반되는 부작용도 적지는 않아[<xref ref-type="bibr" rid="b62-kjm-84-6-781-3">62</xref>,<xref ref-type="bibr" rid="b69-kjm-84-6-781-3">69</xref>] 당뇨병 환자에게는 백신을 개발하여 접종하는 것이 이상적인 치료법이라고 여겨진다[<xref ref-type="bibr" rid="b62-kjm-84-6-781-3">62</xref>]. 제1형 청소년 당뇨병 환자에서 주목할 만한 점은 헬리코박터 파이로리 감염이 동반된 경우 매일 사용하는 인슐린 양이 헬리코박터 파이로리 감염이 없는 군에 비하여 증가한다는 점이다[<xref ref-type="bibr" rid="b72-kjm-84-6-781-3">72</xref>]. 아직까지 헬리코박터 파이로리 감염과 당뇨병의 관계에 대하여서는 명확하지 않은 점이 대부분이지만, 다른 무엇보다 헬리코박터 파이로리 감염 환자에서 혈당의 조절과 제균치료에 관해서 집중적인 연구가 이루어져야하리라고 생각한다.</p> 
</sec>
<sec><title>헬리코박터 파이로리 감염과 지질이상</title>
<p>헬리코박터 파이로리 감염은 총 콜레스테롤, 저밀도지질단백질, 지질단백질 Lp (a), 아포지질단백질 apo-B, 중성지방의 혈중 농도를 올리고, 고밀도지질단백질과 아포지질단백질 apoA-I의 농도를 낮추는 것으로 알려져 있다[<xref ref-type="bibr" rid="b73-kjm-84-6-781-3">73</xref>-<xref ref-type="bibr" rid="b78-kjm-84-6-781-3">78</xref>]. 특히 뇌혈관 질환자에서 저밀도지질단백질은 헬리코박터 파이로리 감염군에서 비감염군에 비하여 월등히 높았다[<xref ref-type="bibr" rid="b74-kjm-84-6-781-3">74</xref>]. 헬리코박터 파이로리 감염이 만성적으로 지속될 때 IL-1 또는 IL-6, IFN-&#x003b1; 그리고 TNF-&#x003b1; 등 염증유발성 사이토카인의 작용이 증가하는데 이러한 사이토카인들은 지방조직에서 지질단백질분해효소의 활성을 증가시키고, 간에서 지방산의 합성과 지질분해를 유도하여 혈중 지질대사는 동맥경화를 유발하는 쪽으로 기울게 된다[<xref ref-type="bibr" rid="b75-kjm-84-6-781-3">75</xref>,<xref ref-type="bibr" rid="b79-kjm-84-6-781-3">79</xref>]. 결과적으로 헬리코박터 파이로리 감염으로 인해 동맥경화가 일어나기 쉬운 환경이 조성되어 심혈관 질환과 뇌혈관 질환이 발생하기 쉬운 상황이 연출되는데 특히 CagA 양성 헬리코박터 파이로리 감염일 경우 이러한 현상이 더욱 극명히 나타날 것이라고 여겨지고 있다[<xref ref-type="bibr" rid="b80-kjm-84-6-781-3">80</xref>,<xref ref-type="bibr" rid="b81-kjm-84-6-781-3">81</xref>]. 물론 헬리코박터 파이로리 감염이 혈중 총 콜레스테롤, 중성지방, 저밀도지질단백질, 그리고 아포지질단백질 apo-B에 영향을 주지 않는다는 보고도 있다[<xref ref-type="bibr" rid="b82-kjm-84-6-781-3">82</xref>,<xref ref-type="bibr" rid="b83-kjm-84-6-781-3">83</xref>]. 헬리코박터 파이로리 제균 후 지질 조성의 변화에 대하여도 서로 다른 의견이 대립하고 있다. Scharnagl 등[<xref ref-type="bibr" rid="b84-kjm-84-6-781-3">84</xref>]은 십이지장 궤양환자에서 헬리코박터 파이로리를 제균한 지 1년 후 고밀도지질단백질과 아포지질단백질 apoA-I 및 apoA-II가 증가하였다고 하였다. Majka 등[<xref ref-type="bibr" rid="b74-kjm-84-6-781-3">74</xref>]도 성공적으로 제균요법을 마친 뒤 6개월 째 혈중 지질을 측정하였을 때 헬리코박터 파이로리 양성군에 비하여 치료군에서 총 콜레스테롤 저밀도지질단백질이 현저히 감소하였다고 보고하였다. 반면에 헬리코박터 파이로리 제균은 혈중지질 농도에 약간의 영향만[<xref ref-type="bibr" rid="b82-kjm-84-6-781-3">82</xref>] 주었거나 오히려 제균치료 후 총 콜레스테롤과 중성지방이 현저히 증가하였다는 보고도 있다[<xref ref-type="bibr" rid="b31-kjm-84-6-781-3">31</xref>,<xref ref-type="bibr" rid="b32-kjm-84-6-781-3">32</xref>,<xref ref-type="bibr" rid="b85-kjm-84-6-781-3">85</xref>]. 이렇듯 현재까지는 헬리코박터 파이로리 감염 혹은 제균과 관련하여 혈중지질의 변화에 대한 결과들이 일정한 경향을 보여주지는 못하고 있다. 그러나 전체적인 흐름은 헬리코박터 파이로리 감염이 만성화되면 만성 염증에 관련된 사이토카인들이 분비되고 이러한 물질의 영향으로 혈중지질의 조성이 동맥경화를 유발하기 쉬운 방향으로 전환된다는 방향이라 여겨진다.</p> 
</sec>
<sec><title>헬리코박터 파이로리 감염과 심혈관 질환</title>
<p>헬리코박터 파이로리 감염이 심혈관 질환의 병태생리에 관여하는 원인 중 하나일 가능성일 수 있다는 것은 널리 알려진 이야기 중의 하나였다. 그러나 헬리코박터 파이로리 감염과 심혈관 질환의 연관성에 대한 연구 결과는 긍정적인 견해와 부정적인 견해가 팽팽히 맞서고 있어 어떠한 결론을 내리기 힘든 상태였다[<xref ref-type="bibr" rid="b86-kjm-84-6-781-3">86</xref>-<xref ref-type="bibr" rid="b88-kjm-84-6-781-3">88</xref>]. 헬리코박터 파이로리 감염이 동맥경화를 촉진시키는 지에 대한 동물실험의 경우도 긍정적인 견해와 부정적인 견해가 함께 피력되어 역시 결론을 내릴 수 없었다[<xref ref-type="bibr" rid="b86-kjm-84-6-781-3">86</xref>-<xref ref-type="bibr" rid="b88-kjm-84-6-781-3">88</xref>]. 그러나 최근의 연구 결과들을 살펴보면 CagA 양성 헬리코박터 파이로리 감염은 심혈관 질환의 이환율을 현저히 증가시키는 것으로 보인다. Jafarzadeh 등[<xref ref-type="bibr" rid="b89-kjm-84-6-781-3">89</xref>]은 심근경색, 불안정협심증에 이환된 환자에서 CagA양성 헬리코박터 파이로리 감염률이 대조군에 비하여 현저히 높았다고 하였다. CagA 양성 헬리코박터 파이로리 감염은 심근경색에서 86.7%, 불안정협심증에서 91.7%, 그리고 대조군에서 58.3%였다. 이와 비슷하게 관상동맥경화가 있는 환자군에서 CagA 항체가 월등히 높게 측정되었다는 내용이 이탈리아에서도 보고되었다[<xref ref-type="bibr" rid="b90-kjm-84-6-781-3">90</xref>]. 이 연구에서 흥미로운 점이 하나 있었는데 CagA 항체의 역가가 높을수록 동맥경화에 이환된 혈관의 길이가 길었다는 것이다[<xref ref-type="bibr" rid="b90-kjm-84-6-781-3">90</xref>]. 이러한 현상은 CagA 양성 헬리코박터 파이로리 감염과 관련된 혈액 내 변화를 연구한 내용을 살펴보면 적절한 해답을 얻을 수 있으리라고 생각한다. 최근에 Huang등[<xref ref-type="bibr" rid="b91-kjm-84-6-781-3">91</xref>]이 보고한 바에 의하면, CagA 양성 헬리코박터 파이로리 감염되었을 때 CRP, 총 콜레스테롤, 저밀도지질단백질, 산화된 저밀도지질단백, 그리고 아포지질단백질이 CagA 음성 헬리코박터 파이로리에 감염된 경우에 비하여 월등히 증가된다고 한다. 헬리코박터 파이로리는 von Willebrand Factor에 부착할 수 있고 Ib 당단백질에 작용하여 혈소판의 부착을 유발하고 종국적으로 혈전을 생성시킨다고 보인다[<xref ref-type="bibr" rid="b92-kjm-84-6-781-3">92</xref>]. 헬리코박터 파이로리 감염과 허혈성심혈관 질환의 관계에 대한 의문점이 제기된 이후로 긍정적인 보고와 회의적인 보고가 맞서왔지만, 최근의 연구결과를 보건대 최소한 CagA 양성 헬리코박터 파이로리 감염은 혈전 형성을 유도함으로써 허혈성심혈관 질환을 유발한다는 쪽으로 이론이 정립되어 간다고 생각한다.</p> 
</sec>
<sec><title>헬리코박터 파이로리 감염과 허혈성 뇌혈관 질환</title>
<p>동맥경화의 유발인자로서 헬리코박터 파이로리 감염이 연구되면서 허혈성뇌혈관 질환과 헬리코박터 파이로리 감염의 연관성도 연구되어 왔다. 일부 소규모 연구[<xref ref-type="bibr" rid="b93-kjm-84-6-781-3">93</xref>-<xref ref-type="bibr" rid="b97-kjm-84-6-781-3">97</xref>]에서는 헬리코박터 파이로리 감염이 동맥경화와 연관된 미세혈관병증 뇌혈관 질환과 관련이 있다고 하였으나 대규모 연구에서는 헬리코박터 파이로리 감염이 허혈성뇌혈관 질환과 연관이 없거나 약간 영향이 있는 정도로 밝혀졌다[<xref ref-type="bibr" rid="b98-kjm-84-6-781-3">98</xref>]. 그런데 최근 Marina 등[<xref ref-type="bibr" rid="b99-kjm-84-6-781-3">99</xref>]에 의하여 비교적 근래에 발표된 바에 따르면 CagA 양성 헬리코박터 파이로리 감염이 있는 경우 허혈성뇌혈관 질환의 재발률이 월등이 높았다는 것이 밝혀졌다. 물론 다른 연구를 통하여 충분한 증거가 뒷받침되어야 하겠으나 헬리코박터 파이로리 감염과 심혈관 질환의 관련성이 점차 입증되어가는 것처럼[<xref ref-type="bibr" rid="b100-kjm-84-6-781-3">100</xref>] 언젠가는 CagA 양성 헬리코박터 파이로리 감염과 허혈성뇌혈관 질환의 관련성도 그 실마리를 찾게 되리라고 생각한다. </p>

<sec sec-type="conclusions">
<title>결 론</title>
<p>헬리코박터 파이로리가 발견된 이래로 &#x0201c;이 특이한 세균이 위장관계 질환 외에 다른 여러 질환을 일으키지 않을까?&#x0201d; 하는 질문을 수많은 연구자들이 던졌었고 역학연구 및 병리학적 근거를 찾기 위한 연구를 지속하여 왔다. 이러한 질문 중 대부분은 아직 명확한 답을 얻지 못하고 있는 실정이다. 과연 헬리코박터 파이로리 감염이 비만을 일으키는지 혹은 제균치료 후 체중이 늘어나는 현상이 헬리코박터 파이로리가 제거된 뒤 그렐린의 증가에 따른 현상인지 아직은 모호한 실정이다. 또한 헬리코박터 파이로리 감염 시 인슐린 저항성은 증가하는 것으로 보이나 당뇨병의 발병에까지 기여한다는 증거는 없다. 다만 비록 뇌혈관 질환과의 관련성은 불명확하나 헬리코박터 파이로리 감염이 만성화 되면 지질조성의 변화를 가져와 동맥경화를 심화시키는 위험인자로서 작용하며 심혈관 질환의 이환율을 높이는 것은 어느 정도 정설로 받아들여지는 것으로 보인다. 병태생리와는 별도로, 몇몇 연구에서 밝혀진 바와 같이 헬리코박터 파이로리 제균은 당뇨병의 혈당조절, 지질대사 이상의 교정, 심혈관 질환의 재발률 저하에 효과를 보인다고 여겨지고 있으므로 이러한 환자에서는 헬리코박터 파이로리 제균을 적극적으로 고려하는 것이 바람직하다고 생각한다. 더욱이 이런 환자에서 재감염의 위험 등을 고려하여 볼 때 헬리코박터 파이로리 백신이 개발된다면 큰 도움이 될 수 있을 것이다. </p>
