<abstract>
<p>간문맥 동맥류는 흔치 않은 질환으로 증상 없이 우연히 발견되는 경우가 많다. 정확한 원인은 아직 밝혀지지 않았으며 다양한 모양을 갖고 있다. 기저 간질환이나 문맥압 항진의 증거가 없는 간문맥 동맥류는 비교적 안정적인 경과를 보이는 것으로 알려져 있다. 따라서 조기 진단 후 추적검사를 통해 크기 변화와 합병증 발생 여부에 대한 관찰이 중요하다. 증상이나 합병증이 발생한 경우 중재적 시술이나 수술적 치료가 필요하다. 저자들은 비특이적인 복부 불편감을 주소로 내원한 환자에서 CT상 간외 간문맥 동맥류를 발견한 1예를 경험하였기에 문헌고찰과 함께 증례를 보고하고자 한다.</p></abstract>
<trans-abstract xml:lang="en"><p>Portal venous aneurysm (PVA) is an unusual venous malformation and its etiology is not fully understood. Most PVAs are diagnosed incidentally by non-invasive abdominal imaging techniques; indeed, recently, PVAs have frequently been discovered in this way. Most PVAs remain stable, without complications, in patients with no clinical symptoms of portal hypertension or intrinsic liver disease. However, if PVA is discovered in these patients by chance, close imaging follow-up of portal vein aneurysms is recommended. In the case of aneurysm growth or the development of complications related to the aneurysm, medical or surgical approaches are advised but there is no consensus on the use of prophylactic antiplatelet or anticoagulation medications. We report a case of incidental PVA identification in a patient without underlying liver disease by routine investigation. A review of the literature is also provided. <title>서 론</title>
<p>간외 문맥 동맥류는 드물게 발견되는 이상 소견이다. 1956년 Barzilai와 Klekner [<xref ref-type="bibr" rid="b1-kjm-85-3-294-11">1</xref>]가 간외 문맥 동맥류를 처음 보고하였고 초음파나 computed tomography (CT), magnetic resonance imaging (MRI) 및 혈관조영술 등 진단 기술의 발달로 보고가 늘고 있으나 국내에서는 췌장두부암으로 오인된 경우 등 3예의 보고가 있을 뿐이다[<xref ref-type="bibr" rid="b2-kjm-85-3-294-11">2</xref>-<xref ref-type="bibr" rid="b4-kjm-85-3-294-11">4</xref>]. 간외 문맥 동맥류는 상장간막정맥과 비장정맥의 융합부위와 주간 문맥에 발생하며[<xref ref-type="bibr" rid="b5-kjm-85-3-294-11">5</xref>] 연령이나 성별과는 무관한 것으로 알려져 있다. 기저 간질환이 없는 환자에서는 특이 증상 없이 우연히 발견되는 경우가 많다. 흔치 않은 질환이기 때문에 병의 자연 경과, 임상양상에 대해 잘 알려져 있지 않으며 치료방법에 대해서도 논란의 여지가 있다.</p>

<p>이에 저자들은 비특이적인 복부 불편감을 주소로 내원한 환자에서 CT상 우연히 간외 문맥 동맥류를 발견한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.</p>
</sec>

<sec sec-type="cases">
<title>증 례</title>
<p><bold>환 자:</bold> 안&#x03007;&#x03007;, 69세, 남자</p>

<p><bold>주 소:</bold> 우상복부 불쾌감</p>

<p><bold>현병력:</bold> 간헐적인 식후 우상복부 불쾌감 치료 원해 내원함.</p>

<p><bold>과거력:</bold> 제대염, 복부외상, 췌장염, 간염 등 특이 병력 없음.</p>

<p><bold>가족력:</bold> 특이 가족력 없음.</p>

<p><bold>사회력:</bold> 흡연력 및 음주력 없음.</p>

<p><bold>이학적 소견:</bold> 내원 당시 혈압 115/80 mmHg, 맥박 78회/분, 호흡 20회/분, 체온 36.7&#x02103;였다. 의식은 명료하였으며 병색은 없었다. 공막의 황달은 없었으며 결막도 창백하지 않았다. 복부 촉진상 우상복부 압통은 없었다.</p>

<p><bold>검사실 소견:</bold> 내원 당시 혈액학적 검사상 혈색소 13.5 g/dL, 적혈구 용적 40.8%, 백혈구 4,390 cell/mm<sup>3</sup>, 혈소판 222,000 cell/mm<sup>3</sup>였으며, AST 24U/L, ALT 21U/L, ALP 155U/L, T-Bil 0.9 mg/dL, HBsAg: Negative, HCV Ab: Negative였다.</p>

<p><bold>복부 컴퓨터 단층 촬영 소견:</bold> 비장정맥과 상장간막정맥의 융합부위에 5.7 &#x000d7; 3.5 &#x000d7; 3.9 cm 크기의 방추형 간외 문맥 동맥류가 관찰되었으며 간내 문맥이나 췌장, 담관계는 이상 소견이 없었다. 담낭에 18 mm 크기의 담석이 있었고 6 mm 크기의 간내 석회화가 있었다(<xref rid="f1-kjm-85-3-294-11" ref-type="fig">Fig. 1</xref>).</p>

<p><bold>치료 및 경과:</bold> 환자가 호소하는 증상이 불규칙하고 경미하며 악화 양상이 없고 정맥류나비장종대, 복수 등 문맥압항진증 합병의 증거가 보이지 않아 CT 등을 통한 정기적 추적과 경과관찰을 계획하였다. </p>

<sec sec-type="discussion">
<title>고 찰</title>
<p>간문맥 동맥류는 간문맥, 상장간동맥, 비문맥접합부의 비장정맥에 생긴 동맥류를 지칭한다[<xref ref-type="bibr" rid="b5-kjm-85-3-294-11">5</xref>]. Doust와 Pearce의 초음파 연구에서는 정상 간문맥 직경은 정상인의 경우 1.5 cm로 보고하였고 간경변증 환자에서도 1.9 cm를 넘지 않음을 보고하였다[<xref ref-type="bibr" rid="b6-kjm-85-3-294-11">6</xref>]. 따라서 2.0 cm 이상 늘어난 문맥을 간문맥 동맥류로 정의한다. 간문맥에 발생한 동맥류는 정맥에 발생한 동맥류의 3%로, 보고된 유병률은 0.43%이다[<xref ref-type="bibr" rid="b7-kjm-85-3-294-11">7</xref>]. 1956년 Brazilai와 Klekner [<xref ref-type="bibr" rid="b1-kjm-85-3-294-11">1</xref>]의 간외 문맥 동맥류의 첫 증례보고 후 영상 진단 기술의 발달로 전 세계적으로 그 보고가 증가하는 추세이다. 성별과 관계없으며 모든 연령대에서 발생할 수 있다. 간문맥에 발생한 동맥류는 모양에 따라 방추형, 낭상형, 이소엽형 등으로 분류하는데[<xref ref-type="bibr" rid="b3-kjm-85-3-294-11">3</xref>] 본 증례의 경우 방추형 동맥류로 진단되었다.</p>

<p>발생 원인은 정확히 밝혀지지 않았으나 몇 가지 가설들이 유력하다. 태생기 4-5주 경 간문맥 형성시기에 우측 난황정맥의 불완전 퇴화나 혈관벽의 약화에 의해 선천성 간문맥 동맥류가 발생할 수 있겠다. Gallagher 등[<xref ref-type="bibr" rid="b8-kjm-85-3-294-11">8</xref>]은 산전초음파로 선천성 간문맥 동맥류 발견을 보고한 바 있다. 또, 기저 간질환에 의해 간문맥 압력이 증가하면 혈관벽 약화로 이어져 동맥류가 발생할 수 있다. 그러나 간경변이 확인된 환자에서 간문맥 동맥류 발생보고는 드문 편이어서 일부 연구자들은 선천적인 혈관벽 약화나 문맥 발달과정 중 기형을 가진 환자에서 문맥압 증가로 동맥류 팽창이 나타난다고 주장하기도 한다[<xref ref-type="bibr" rid="b9-kjm-85-3-294-11">9</xref>]. 다른 원인들로는 췌장염에 의한 염증 반응의 합병 및 외상 등이 있다. 본 증례의 경우 특이질환의 과거력, 외상병력이 없고 기저 간질환 및 문맥압 항진의 증거가 없으므로 선천성 간문맥 동맥류가 의심된다.</p>

<p>대부분의 간문맥 동맥류 환자는 증상이 없다. 그러나 크기가 큰 동맥류의 경우 압박에 의해 황달이나 십이지장 폐색이 나타날 수 있고 간질환 및 문맥압 항진증이 동반된 경우 위장관 출혈 및 혈전증 등이 나타날 수 있다. 간외 간문맥에 발생한 동맥류는 초음파, CT, MRI 등의 영상 검사를 통해 쉽게 진단될 수 있다. 문맥압 항진증이 동반되지 않은 간문맥 동맥류의 경우 대부분 장기간의 추적검사에서 크기의 증가나 합병증의 발생이 드문 것으로 알려져 있다. 실제 6 cm의 낭상형 간문맥 동맥류 환자에서 수년간의 추적검사결과 자연 퇴축이 관찰된 예도 있었다[<xref ref-type="bibr" rid="b10-kjm-85-3-294-11">10</xref>]. 따라서 문맥압 항진의 증거가 없을 경우 추적검사로 간문맥 동맥류의 크기 변화나 합병증 발생 여부를 관찰하는 것이 중요하겠다. 간외 간문맥 동맥류의 합병증으로는 혈전증, 담관 폐색, 하대정맥폐색, 십이지장 폐색, 동맥류 파열 등이 있다. 간문맥 동맥류내 혈전증이 발생한 경우 항응고제 치료나 경피적 중재시술을 고려할 수 있다. 극심한 복통이나 압박에 의한 증상, 추적 검사상 크기 증가, 혈전증이나 파열의 위험성이 높은 경우에는 우회로 조성술 또는 동맥류 봉합술 등의 수술적 치료를 필요로 한다. 그러나 수술 시기나 방법 및 그 효과에 대해서는 아직 논란의 여지가 있다. 본 증례의 경우 기저 간질환 및 문맥압 항진증이 없고 압박 증상도 뚜렷하지 않았으며 혈전증 등 합병증 소견도 확인되지 않아 정기적인 추적검사가 중요할 것으로 사료된다.</p>
