<article-title>필라델피아 염색체-유사 급성 림프모구백혈병의 치료 전략</article-title>

<trans-abstract xml:lang="en"><p>Philadelphia chromosome-like acute lymphoblastic leukemia (ALL) is a group of diseases that possess similar gene expression patterns to those of Philadelphia chromosome-positive ALL, but lack the <italic>BCR-ABL1</italic> gene rearrangement and exhibit diverse genetic characteristics. These cases are associated with a poorer prognosis than that of other B-cell ALL cases and require novel therapeutic approaches. Philadelphia chromosome-like ALL can be categorized into ABL-activated, Janus kinase-signal transducer and activator of transcription (JAK-STAT) pathway-altered, and other subtypes based on genetic alterations. For the ABL-activated subtype, treatment with ABL-directed tyrosine kinase inhibitors may improve outcomes, whereas JAK-directed tyrosine kinase inhibitors have been proposed as a combination therapy for patients with JAK-STAT pathway mutations. In this review, we discuss treatment approaches for each subtype of Philadelphia chromosome-like ALL.</p></trans-abstract>

<title>서 론</title>
<p>필라델피아 염색체-유사(Philadelphia chromosome-like) 급성 림프모구백혈병, 다른 표현으로는 <italic>BCR-ABL1</italic>-유사 급성 림프모구백혈병은 필라델피아 염색체-양성 급성 림프모구백혈병과 유사한 유전자 발현 패턴을 보이지만 <italic>BCR-ABL1</italic> 유전자 재배열이 없는 질환군을 일컫는다&#x005B;<xref ref-type="bibr" rid="b1-kjm-100-2-75">1</xref>&#x005D;. 이것은 2009년에 처음으로 소개되었으며 독일과 네덜란드의 급성 림프모구백혈병 소아 환자 297명을 대상으로 110개의 유전자의 발현을 분석한 연구 결과에서 <italic>BCR-ABL1</italic> 유전자 재배열이 있는 환자와 유사한 유전자 발현 패턴을 가지고 있는 환자군이 관찰되어 이를 필라델피아 염색체-유사 또는 <italic>BCR-ABL1</italic>-유사 급성 림프모구백혈병이라 명명하였다&#x005B;<xref ref-type="bibr" rid="b2-kjm-100-2-75">2</xref>&#x005D;. 같은 해 미국의 Children&#x02019;s Oncology Group (COG)에서 221명의 소아 급성 림프모구백혈병 환자 코호트를 대상으로 시행한 연구에서는 이카로스 계열 아연 손가락 단백질 1 (Ikaros family zinc finger protein 1, <italic>IKZF1</italic>) 유전자의 결손 혹은 돌연변이가 있는 급성 림프모구백혈병 환자의 예후가 매우 불량함을 보고하였고 <italic>IKZF1</italic> 유전자의 변형이 필라델피아 염색체-유사 급성 림프모구백혈병 환자에서 빈번히 나타남을 확인하였다&#x005B;<xref ref-type="bibr" rid="b2-kjm-100-2-75">2</xref>,<xref ref-type="bibr" rid="b3-kjm-100-2-75">3</xref>&#x005D;.</p>
<p>필라델피아 염색체-유사 급성 림프모구백혈병은 다른 B-세포 급성 림프모구백혈병에 비하여 예후가 불량하며 소아를 대상으로 한 연구에서 다른 B-세포 급성 림프모구백혈병의 5년 무병 생존율(disease-free survival)이 84%였던 반면 필라델피아 염색체-유사 급성 림프모구백혈병 환자에서는 59%에 불과하였으며 이는 필라델피아 염색체-양성 급성 림프모구백혈병과 유사한 결과였다&#x005B;<xref ref-type="bibr" rid="b2-kjm-100-2-75">2</xref>&#x005D;. 성인 환자를 대상으로 한 연구에서도 필라델피아 염색체-유사 급성 림프모구백혈병 환자의 5년 무사건 생존율(event-free survival)은 22.5%로 다른 B-세포 급성 림프모구백혈병의 49.3%에 비하여 통계적으로 의미 있게 낮았다&#x005B;<xref ref-type="bibr" rid="b4-kjm-100-2-75">4</xref>&#x005D;. 필라델피아 염색체-유사 급성 림프모구백혈병 환자가 불량한 예후를 가지는 이유로는 필라델피아 염색체-유사 급성 림프모구백혈병 세포가 세포독성 항암제, 특히 L-아스파라기나제(L-asparaginase)와 다우노루비신(daunorubicin)에 내성을 가지는 <italic>in vitro</italic> 실험 결과가 제시된다&#x005B;<xref ref-type="bibr" rid="b2-kjm-100-2-75">2</xref>&#x005D;.</p>
<p>필라델피아 염색체-유사 급성 림프모구백혈병의 발병 기전은 여전히 연구 중이긴 하나 Janus kinase-signal transducer and activator of transcription (JAK-STAT) 경로를 경유하는 사이토카인 수용체 신호(cytokine receptor signaling)의 상승으로 인하여 세포의 증식이 촉진되지만 B-세포 분화에 작용하는 전사인자(transcriptional factor)인 <italic>IKZF1</italic>, <italic>BTG1</italic>, <italic>EBF1</italic>, <italic>PAX5</italic>와 같은 유전자의 돌연변이로 인해 B-세포가 성숙하지 못하여 암세포로 변화되는 것으로 설명된다&#x005B;<xref ref-type="bibr" rid="b5-kjm-100-2-75">5</xref>,<xref ref-type="bibr" rid="b6-kjm-100-2-75">6</xref>&#x005D;. 필라델피아 염색체-유사 급성 림프모구백혈병은 유전자 발현 패턴에 따라 다시 5가지로 구분할 수 있으며 1) ABL1 활성화 부류(class), 2) JAK-STAT 경로 변이, 3) 사이토카인수용체 유사 인자 2 (cytokine receptor like factor 2 &#x005B;<italic>CRLF2</italic>&#x005D;) 유전자 과발현, 4) RAS 경로 변이, 5) 기타(other) 변이가 그것이다&#x005B;<xref ref-type="bibr" rid="b7-kjm-100-2-75">7</xref>&#x005D;. 필라델피아 염색체-유사 급성 림프모구백혈병은 2016년의 제4판 국제보건기구(World Health Organization, WHO) 분류에서는 임시적인 질환 단위로 구분하였지만 2022년 제5판 WHO 분류에서는 하나의 확정적인 질환 단위로 규정하고 있다&#x005B;<xref ref-type="bibr" rid="b8-kjm-100-2-75">8</xref>,<xref ref-type="bibr" rid="b9-kjm-100-2-75">9</xref>&#x005D;. 또한 2022년의 국제합의분류(International Consensus Classification, ICC)에서도 <italic>BCR-ABL1</italic>-유사 급성 림프모구백혈병을 3가지의 질환, 즉 ABL1 부류 재배열, JAK-STAT 활성화 그리고 기타로 구분하여 제시하고 있어 이 질환군의 임상적 중요성이 높아지고 있다&#x005B;<xref ref-type="bibr" rid="b10-kjm-100-2-75">10</xref>&#x005D;.</p>
<p>최근에는 필라델피아 염색체-유사 급성 림프모구백혈병의 발병 기전이 밝혀지면서 특정 유전자 돌연변이를 표적으로 하는 치료제에 대한 연구 결과가 보고되고 있다. 본고에서는 최근 문헌을 검토하여 이에 대해 설명하고자 한다.</p>

<title>본 론</title>
<sec>
<title>필라델피아 염색체-유사 급성 림프모구백혈병의 진단</title>
<p>필라델피아 염색체-유사 급성 림프모구백혈병은 소아 급성 림프모구백혈병의 약 10-20%를 차지하고 20대에서 40대 사이의 성인에서는 발생률이 20-30% 정도로 증가한다&#x005B;<xref ref-type="bibr" rid="b2-kjm-100-2-75">2</xref>,<xref ref-type="bibr" rid="b3-kjm-100-2-75">3</xref>&#x005D;. 필라델피아 염색체-양성 급성 림프모구백혈병은 연령이 증가함에 따라 발생률이 지속적으로 증가하지만 필라델피아 염색체-유사 급성 림프모구백혈병은 20-40대에서 발생률이 정점을 이루고 이후 점차 감소하는 것으로 알려져 있다&#x005B;<xref ref-type="bibr" rid="b11-kjm-100-2-75">11</xref>&#x005D;.</p>
<p>필라델피아 염색체-유사 급성 림프모구백혈병의 진단을 위해서는 먼저 <italic>BCR-ABL1</italic>, <italic>KMT2A-</italic>, <italic>ETV-RUNX1</italic>, <italic>TCF3-PBX1</italic> 재배열 변이가 있는지 확인하여야 한다&#x005B;<xref ref-type="bibr" rid="b12-kjm-100-2-75">12</xref>&#x005D;. 이러한 유전자 재배열이 없을 때 필라델피아 염색체-유사 급성 림프모구백혈병을 진단하기 위한 절차가 시작된다. 하지만 필라델피아 염색체-유사 급성 림프모구백혈병의 진단을 최적화하기 위한 노력은 여전히 진행 중이다. 초창기에는 257개의 유전자 세트를 이용하여 필라델피아 염색체-유사 급성 림프모구백혈병의 특징적인 유전자 발현 패턴을 분석하였지만 비용 효과성을 고려하여 최근에는 15개의 유전자를 선별하여 검사하는 저밀도 마이크로어레이(low density microarray)를 먼저 추천한다&#x005B;<xref ref-type="bibr" rid="b12-kjm-100-2-75">12</xref>,<xref ref-type="bibr" rid="b13-kjm-100-2-75">13</xref>&#x005D;. 저밀도 마이크로어레이는 257개의 유전자 세트 검사와 비교하였을 때 약 87%에서 일치하는 결과가 나온다&#x005B;<xref ref-type="bibr" rid="b13-kjm-100-2-75">13</xref>&#x005D;. 저밀도 마이크로어레이를 통해 <italic>CRLF2</italic> 유전자가 과발현되어 있으면 <italic>CRLF2</italic>에 대한 형광제자리부합법(fluorescence in situ hybridization, FISH) 검사를 시행하고 JAK2 유전자 돌연변이를 검사한다&#x005B;<xref ref-type="bibr" rid="b1-kjm-100-2-75">1</xref>,<xref ref-type="bibr" rid="b12-kjm-100-2-75">12</xref>,<xref ref-type="bibr" rid="b14-kjm-100-2-75">14</xref>&#x005D;. 그리고 저밀도 마이크로어레이 검사에서 <italic>CRLF2</italic> 과발현이 없을 경우 <italic>ABL1</italic>, <italic>CSF1R</italic>, <italic>PDGFRA</italic>, <italic>PDGFRB</italic>, <italic>CRLF2</italic>, <italic>JAK2</italic>, <italic>EPOR</italic> 등의 유전자에 대한 재배열 여부를 FISH로 확인한 후 재배열이 관찰되면 필라델피아 염색체-유사 급성 림프모구백혈병으로 진단할 수 있고 그렇지 않으면 리보핵산(ribonucleic acid, RNA) 염기서열 분석을 이용하여 다른 유전자의 재배열을 확인한다. 국내에서는 필라델피아 염색체-유사 급성 림프모구백혈병의 진단을 위한 검사가 각 기관에 따라 상이하여 향후 이를 표준화하기 위한 노력이 필요한 상태이다.</p>

<sec>
<title>ABL 활성화 부류(class)</title>
<p>ABL 활성화 부류는 ABL 유전자 재배열과 유사한 유전자 발현 패턴을 가지는 그룹이며 <italic>ABL1, ABL2, CSF1R, PDGFRA, PDGFRB, LYN</italic> 유전자 결합(fusion)이 여기에 속한다&#x005B;<xref ref-type="bibr" rid="b7-kjm-100-2-75">7</xref>&#x005D;. ABL 활성화 부류는 전체 B-세포 급성 림프모구백혈병의 약 2-3%를 차지하며 필라델피아 염색체-유사 급성 림프모구백혈병 환자에서는 약 10-15%를 차지하는 것으로 알려져 있다&#x005B;<xref ref-type="bibr" rid="b1-kjm-100-2-75">1</xref>&#x005D;. 현재 약 30여 개의 결합 파트너(fusion partner) 유전자가 확인된 상태이며 이 중 <italic>PDGFRB</italic> 유전자 결합이 ABL 활성화 부류의 약 50%에 해당한다&#x005B;<xref ref-type="bibr" rid="b15-kjm-100-2-75">15</xref>,<xref ref-type="bibr" rid="b16-kjm-100-2-75">16</xref>&#x005D;. 비교적 빈번히 관찰되는 유전자 변이로는 5번 염색체 장완에 존재하는 <italic>EBF1</italic> 유전자와 <italic>PDGFRB</italic> 유전자가 결합되는 것이 있으며 이러한 유전자의 재배열으로 인하여 세포 내 신호전달 체계 중 세포의 증식과 관련된 Stat5, Akt, Erk1/2 등이 활성화된다&#x005B;<xref ref-type="bibr" rid="b15-kjm-100-2-75">15</xref>,<xref ref-type="bibr" rid="b17-kjm-100-2-75">17</xref>&#x005D;. ABL 활성화 부류는 예후가 매우 불량한 것으로 알려져 있으며 특히 <italic>EBF1-PDGFRB</italic> 유전자 재배열이 있을 경우에는 관해 유도 항암 화학 치료의 실패율이 높고 완전 관해에 도달하더라도 미세 잔존 질환이 남아 있는 경우가 많은 것으로 보고되고 있다&#x005B;<xref ref-type="bibr" rid="b18-kjm-100-2-75">18</xref>-<xref ref-type="bibr" rid="b20-kjm-100-2-75">20</xref>&#x005D;. 이러한 ABL 활성화 부류의 치료에 이마티닙(imatinib) 혹은 다사티닙(dasatinib)과 같은 ABL 단백질을 표적으로 하는 타이로신키나아제억제제(tyrosine kinase inhibitor)가 도입되어 치료 성적이 향상되고 있다&#x005B;<xref ref-type="bibr" rid="b20-kjm-100-2-75">20</xref>-<xref ref-type="bibr" rid="b22-kjm-100-2-75">22</xref>&#x005D;. 유럽에서 다기관으로 후향적 분석을 시행한 연구에 의하면 ABL 부류의 재배열이 있는 24명의 환자를 대상으로 이마티닙 혹은 다사티닙을 투여하였을 때 기존 항암 치료에 불응 상태였던 7명 중 6명에서 완전 관해가 획득되었고 미세 잔존 질환이 있던 18명 중에서는 14명(78%)에서 미세 잔존 질환이 없어지는 것이 관찰되었다&#x005B;<xref ref-type="bibr" rid="b23-kjm-100-2-75">23</xref>&#x005D;. 그리고 3년 무사건 생존율(event-free survival)과 전체 생존율(overall survival)이 각각 55%와 77%로 기존에 알려진 생존율에 비하여 우월한 결과가 관찰되었다&#x005B;<xref ref-type="bibr" rid="b23-kjm-100-2-75">23</xref>&#x005D;. 그리하여 현재 미국의 COG에서는 다사티닙과 블리나투모맙(blinatumomab)을 병행하여 투여하는 임상 시험이 ABL 부류 재배열을 가진 필라델피아 염색체-유사 급성 림프모구백혈병 환자를 대상으로 진행 중이다(NCT06124157). 한편 최근 포나티닙(ponatinib)에 대한 후향적 연구가 이탈리아 연구자들에 의하여 발표되었다. ABL 활성화 부류뿐만 아니라 <italic>CRLF2</italic> 재배열이 있는 환자들도 포함되었다는 것이 이 연구의 단점이긴 하지만 여기서도 약 56%의 환자들이 포나티닙 단독 혹은 병합 요법으로 부분 반응 이상을 보였다&#x005B;<xref ref-type="bibr" rid="b24-kjm-100-2-75">24</xref>&#x005D;. 따라서 향후 ABL 활성화 부류 환자들만을 대상으로 한 포나티닙의 임상시험이 필요할 것으로 생각된다.</p>
</sec>
<sec>
<title>JAK-STAT 경로 변이</title>
<p>JAK-STAT 신호전달 경로는 세포 내 단백질 사이의 일련의 상호작용으로 세포의 분열과 증식 혹은 면역작용 등에 관여한다&#x005B;<xref ref-type="bibr" rid="b25-kjm-100-2-75">25</xref>&#x005D;. 필라델피아 염색체-유사 급성 림프모구백혈병에서 JAK-STAT 경로의 변이는 크게 <italic>CRLF2</italic> 유전자가 과발현되는(overexpressed) 집단과 그렇지 않은 집단으로 나눌 수 있다&#x005B;<xref ref-type="bibr" rid="b26-kjm-100-2-75">26</xref>&#x005D;. <italic>CRLF2</italic> 과발현은 필라델피아 염색체-유사 급성 림프모구백혈병 환자의 약 50% 정도에서 발견되며 <italic>CRLF2</italic> 과발현되어 있지 않은 JAK-STAT 경로 변이 환자는 필라델피아 염색체-유사 급성 림프모구백혈병 환자의 약 20-30% 정도를 차지한다&#x005B;<xref ref-type="bibr" rid="b1-kjm-100-2-75">1</xref>,<xref ref-type="bibr" rid="b27-kjm-100-2-75">27</xref>&#x005D;. <italic>CRLF2</italic> 과발현은 다운증후군과 연관된 급성 림프모구백혈병에서 빈번히 발견되며 <italic>JAK1</italic> 또는 <italic>JAK2</italic> 유전자의 돌연변이가 동반되는 경우 또한 빈번한 것으로 알려져 있다&#x005B;<xref ref-type="bibr" rid="b26-kjm-100-2-75">26</xref>,<xref ref-type="bibr" rid="b28-kjm-100-2-75">28</xref>&#x005D;. 그리고 B세포의 분화와 관련된 유전자인 <italic>IKZF1</italic>, <italic>CDKN2A/B</italic> 그리고 <italic>PAX5</italic>의 결손도 흔히 관찰된다&#x005B;<xref ref-type="bibr" rid="b26-kjm-100-2-75">26</xref>&#x005D;. <italic>CRLF2</italic> 유전자가 과발현되는 되는 기전으로는 X 또는 Y 염색체에 존재하는 <italic>CRLF2</italic> 유전자가 염색체의 중간 결손(interstitial deletion)에 의하여 같은 염색체 안에 있는 다른 유전자, 예를 들어 <italic>P2RY8</italic>, <italic>CSF2RA</italic> 등과 재배열되는 것과 14번 염색체에 있는 <italic>IGH</italic> 유전자와 <italic>CRLF2</italic>가 재배열됨으로써 발생하는 것이 있다. 또한 매우 드물게는 <italic>CRLF2</italic> 유전자의 점돌연변이(point mutation)에 의해서도 <italic>CRLF2</italic> 과발현이 발생하는 것으로 알려져 있다&#x005B;<xref ref-type="bibr" rid="b26-kjm-100-2-75">26</xref>&#x005D;. <italic>CRLF2</italic> 과발현은 그렇지 않은 집단에 비하여 예후가 매우 불량한 것으로 알려져 있다. MD 앤더슨 암 센터(MD Anderson Cancer Center)에서 148명의 성인 급성 림프모구백혈병 환자를 대상으로 시행한 연구 결과를 보면 필라델피아 염색체-유사 급성 림프모구백혈병 환자의 5년 생존율은 23%였고 <italic>CRLF2</italic> 과발현이 있는 집단의 5년 생존율은 20%에 미치지 못할 정도로 예후가 매우 불량하였다&#x005B;<xref ref-type="bibr" rid="b29-kjm-100-2-75">29</xref>&#x005D;.</p>
<p>JAK-STAT 변이가 있는 필라델피아 염색체-유사 급성 림프모구백혈병에서는 JAK 단백질을 표적으로 하는 타이로신키나아제억제제가 치료제로 효과가 있음이 제시되고 있다&#x005B;<xref ref-type="bibr" rid="b30-kjm-100-2-75">30</xref>,<xref ref-type="bibr" rid="b31-kjm-100-2-75">31</xref>&#x005D;. 한 증례 보고에 의하면 <italic>JAK2-GOLGA5</italic> 재배열이 있는 필라델피아 염색체-유사 급성 림프모구백혈병 환자에게 JAK 억제제인 룩솔리티닙(ruxolitinib)을 투여하였을 때 미세 잔존 질환이 음성으로 전환되는 효과가 있었다&#x005B;<xref ref-type="bibr" rid="b32-kjm-100-2-75">32</xref>&#x005D;. 이와 같이 JAK-STAT 변이가 있는 경우에는 룩솔리티닙을 추가하는 것이 일부 효과가 있을 수 있지만 MD 앤더슨 암 센터에서 수행한 임상시험(NCT02420717)에서는 필라델피아 염색체-유사 급성 림프모구백혈병에서 룩솔리티닙을 추가하는 것의 효과를 입증하는 데 실패하여 조기 종료되기도 하였다. 그래서 JAK-STAT 변이가 있을 때 룩솔리티닙을 추가하는 것은 향후 추가적인 연구를 통해 검증이 필요한 상태이다.</p>
<p>JAK-STAT 변이가 있는 필라델피아 염색체-유사 급성 림프모구백혈병 환자에서 룩솔리티닙의 효과가 낮음을 설명하기 위하여 수행한 전임상연구에서는 JAK-STAT 변이에 <italic>JAK</italic> 유전자의 돌연변이가 동반되었을 때에는 JAK-STAT 경로를 우회하는 CRKL 경로가 활성화되어 JAK 억제제의 효과를 떨어뜨린다고 제시하였다&#x005B;<xref ref-type="bibr" rid="b33-kjm-100-2-75">33</xref>&#x005D;. 이 연구들은 이렇게 우회하는 경로가 활성화되었을 때에는 CRKL 경로를 억제할 수 있는 약제인 길테리티닙(gilteritinib)을 추가하였을 때 효과적으로 암세포의 사멸을 유도할 수 있다고 보고하였다&#x005B;<xref ref-type="bibr" rid="b33-kjm-100-2-75">33</xref>&#x005D;. 그리하여 향후 <italic>JAK</italic> 유전자 돌연변이를 동반한 JAK-STAT 변이가 있는 필라델피아 염색체-유사 급성 림프모구백혈병 환자의 치료를 위하여 룩솔리티닙과 다른 신호전달 경로를 억제할 수 있는 약제(길테리티닙 등)를 병용 투여하는 것에 대한 임상 연구가 필요할 것이다.</p>
<p>한편 최근에는 JAK-STAT 변이가 있는 필라델피아 염색체-유사 급성 림프모구백혈병 환자의 치료에 BCR2 억제제인 베네토클락스(venetoclax)를 추가하는 것에 대한 연구가 보고되고 있다. 베네토클락스는 항-사멸(anti-apoptotic) 단백질인 BCR2에 부착하여 그 작용을 차단함으로써 암세포의 사멸을 유발하는 약제로 현재 급성 골수백혈병과 만성 림프구백혈병에서 널리 사용되고 있는 약제이다. 한 증례 보고에서는 JAK-STAT 변이가 있는 재발된 필라델피아 염색체-유사 급성 림프모구백혈병 환자에게 베네토클락스를 투여하여 장기간 반응이 유지됨을 보고하였다&#x005B;<xref ref-type="bibr" rid="b34-kjm-100-2-75">34</xref>&#x005D;. 또한 필라델피아 염색체-유사 급성 림프모구백혈병을 포함한 성인 환자 14명을 대상으로 한 1상 임상시험에서는 기존 항암 화학 요법에 베네토클락스를 추가하였을 때 11명(79%)에서 관해 유도 요법 후 미세 잔존 질환이 관찰되지 않았고 관해 유도 요법 및 공고 요법 도중에 사망한 환자가 없어서 독성도 증가하지 않음을 보고하였다&#x005B;<xref ref-type="bibr" rid="b35-kjm-100-2-75">35</xref>&#x005D;. 그리하여 향후 JAK-STAT 변이가 있는 필라델피아 염색체-유사 급성 림프모구백혈병 환자의 치료에 베네토클락스를 병합하는 것의 임상시험 결과가 기대된다.</p>
</sec>
<sec>
<title>기타(others)</title>
<p><italic>ABL</italic> 활성화 부류 혹은 JAK-STAT 경로 변이에 속하지 않는 필라델피아 염색체-유사 급성 림프모구백혈병 환자 약 10%는 기타 집단으로 분류된다. 여기에는 RAS 경로 변이, 예를 들어 <italic>FLT3, NRAS, KRAS, BRAF, PTPN11, NF1</italic> 유전자 변이 등이 속한다&#x005B;<xref ref-type="bibr" rid="b1-kjm-100-2-75">1</xref>&#x005D;. RAS 경로를 억제할 수 있는 약제로는 FLT3 억제제나 MEK 억제제가 제시되고 있으며 아직 임상에서 적용된 적은 없기 때문에 향후 임상시험이 필요한 상태이다. 한 전임상 연구에 의하면 <italic>RAS</italic> 돌연변이가 있는 필라델피아 염색체-유사 급성 림프모구백혈병 환자의 암세포에 MEK 억제제인 트라메티닙(trametinib)을 투여하였을 때 암세포가 효과적으로 사멸하는 것이 관찰되었다. 따라서 향후 이러한 약제를 이용한 임상시험이 필요할 것이다&#x005B;<xref ref-type="bibr" rid="b33-kjm-100-2-75">33</xref>&#x005D;.</p>
</sec>
<sec>
<title>기존의 항암 화학 요법과 조혈모세포 이식</title>
<p>필라델피아 염색체-유사 급성 림프모구백혈병 환자에게 적합한 기존의 항암 화학 요법은 아직 정립되지 않았다. 다만 항암 화학 요법을 선택할 때 환자의 연령을 고려하는 것이 필요하다&#x005B;<xref ref-type="bibr" rid="b36-kjm-100-2-75">36</xref>&#x005D;. 필라델피아 염색체-유사 급성 림프모구백혈병은 소아와 젊은 성인에서 호발하는데 이러한 연령의 환자에게는 소아 환자의 항암 화학 요법(pediatric inspired regimen)으로 치료를 하는 것이 추천된다&#x005B;<xref ref-type="bibr" rid="b37-kjm-100-2-75">37</xref>,<xref ref-type="bibr" rid="b38-kjm-100-2-75">38</xref>&#x005D;. 그리고 노인이거나 기저 질환이 있는 환자에게는 저강도의 항암 화학 요법을 시행하는 것이 좋겠다&#x005B;<xref ref-type="bibr" rid="b39-kjm-100-2-75">39</xref>&#x005D;. 또한 필라델피아 염색체-유사 급성 림프모구백혈병은 재발의 위험성이 높고 예후가 불량하므로 완전 관해에 도달한 환자의 경우에는 동종 조혈모세포 이식을 시행하는 것이 권장된다&#x005B;<xref ref-type="bibr" rid="b40-kjm-100-2-75">40</xref>&#x005D;.</p>
</sec>
</sec>
<sec sec-type="conclusions">
<title>결 론</title>
<p>필라델피아 염색체-유사 급성 림프모구백혈병은 성인 급성 림프모구백혈병의 약 20-30%를 차지하며 예후가 매우 불량한 아류형(subtype)이다. ABL을 표적으로 하는 약제와 JAK-STAT 경로를 표적으로 하는 약제들이 현재 다양한 증례 보고에서 효과를 나타내고 있으며 임상시험이 진행 중이다. 우리나라에서도 이러한 필라델피아 염색체-유사 급성 림프모구백혈병 환자에게 표적 치료제를 추가하여 사용하는 항암 요법의 도입이 필요한 상태이며 향후 급성 림프모구백혈병 연구회를 통해서 활발한 임상 연구가 진행되어야 하겠다.</p>
