Fludarabine monophosphate is a chemotherapeutic agent of purine analogue for refractory chronic lymphocytic leukemia. A 60-year-old man presented with fever and cough about 2 weeks following the first cycle of fludarabine therapy for chronic lymphocytic leukemia. Chest radiologic findings showed patch and extensive ground-glass appearance in both lung fields. Histopathologic findings of lung lesion demonstrated loose granuloma in peribronchiolar interstitium. His symptoms and signs resolved spontaneously with time. To our knowledge, this is the first report on fludarabine-induced pulmonary disease in Korea. Fludarabine-induced lung disease must be considered in all patients who develop unexplained lung disease while receiving fludarabine.(Korean J Med 70:S227-S230, 2006)
