Xanthogranulomatous pyelonephritis is an atypical form of chronic bacterial pyelonephritis which results in the destruction of the renal parenchyma that is replaced by granulomatous tissue containing lipid-laden macrophages. We report a case of a spontaneous rupture of xanthogranulomatous pyelonephritis in a hemodialysis patient. A twenty five-year-old female suddenly presented with severe pain in the left abdomen in the absence of a prior history of trauma or infection. She received hemodialysis for one year because of diabetic renal failure. Abdomen-pelvic CT revealed a cyst and mass with a perirenal hematoma in the left kidney. A left total nephrectomy was performed under the presumptive diagnosis of spontaneous kidney rupture. The pathological findings were compatible with xanthogranulomatous pyelonephritis including the formation of a ruptured abscess.(Korean J Med 70:S294-S298, 2006)
