Abstract
Introduction : Inflammatory mechanisms play a crucial role in the pathogenesis of neointimal hyperplasia (NIH) after coronary stenting. Although DES (Drug-eluting stent) reduce restenosis by inhibiting NIH, two DES, paclitaxel-eluting stent (PES) and silorimus-eluting stent (SES), showed different effect on suppression of NIH in several studies. So we compared plasma C-reactive protein (CRP) levels after PES and SES implantation. Method : We performed percutaneous coronary intervention with a single stent in 63 patients (44 men; 57.4¡¾9,7 years of age; PES group, n=31; SES group, n=32) who had stable angina. Plasma CRP levels were determined before intervention and at 24 hours, 72 hours, and 4 weeks after coronary stenting. Results : There was no difference in clinical and angiographic baseline characteristics. Plasma CRP levels at baseline were not different between both groups (SES group : 6.90¡¾7.46 vs PES group : 6.73¡¾9.08 mg/L, p=0.941). Although plasma CRP levels at 24 and 72 hours after stent implantation were higher in the PES than in the SES group, there was no statistical significance (11.62¡¾14.42 vs 9.30¡¾10.07 mg/L, p=0.527; 11.05¡¾12.10 vs 8.64¡¾6.68 mg/L, p=0.406, respectively). Plasma CRP levels at 4 weeks after stent implantation were not different between both groups. Conclusions : In conclusion, SES did not show significantly lower plasma CRP levels after coronary stenting compared with PES. This study suggested that other factors, such as mechanical or pharmacokinetic properties rather than anti-inflammatory reaction of DES, may play a more crucial role in suppression of NIH after DES implantation.
