A case of multiple endocrine neoplasia type 1 with pulmonary large cell neuroendocrine carcinoma

Abstract
Multiple endocrine neoplasia type 1 (MEN 1) is an autosomal- ominant inherited syndrome that's characterized by the combined occurrence of tumors of the parathyroid glands, endocrine pancreas and anterior pituitary gland. Pulmonary large cell neuroendocrine carcinoma and colon leiomyoma in patients with MEN 1 are rare findings. Here we report a 51-year-old man who was admitted because of a solitary pulmonary nodule that was seen on the whole body MRI. The laboratory results showed hypercalcemia and hypophosphatemia, and the hormonal studies revealed an elevated level of intact parathyroid hormone (iPTH). The 99mTc-sestamibi scintigraphy showed three hot-uptake lesions in the lower neck, and colonoscopy revealed two polyps. Right middle lobe lobectomy was performed and pulmonary large cell neuroendocrine carcinoma was comfirmed. Both parathyroidectomy and colonscopic polypectomy were performed and parathyroid hyperplasia and colon leiomyoma were confirmed.
