- Abstract
- Purpose
: To determine the activity and the toxicities associated with a low dose leucovorin (ldLV) plus 5-fluorouracil (5-FU)
regimen combined with irinotecan every two weeks (modified FOLFIRI), as a salvage therapy for patients with advanced gastric
cancer. Patients and Methods : Between September 2004 and June 2006, thirty-six patients were enrolled in this study. Patients
were treated by irinotecan 150 mg/m2 (2-hour infusion) on day 1 and received LV 20 mg/m2 (over 10 minutes) followed by
5-FU 400 mg/m2 (bolus) and 5-FU 600 mg/m2 (22- hour continuous infusion) on days 1 and 2 every 14 days. Results : Median
patient age was 55 years (range: 31-70), 55.6% (20/36) had an Eastern Cooperative Oncology Group performance status of 0 and
1. Of the 30 patients evaluable for tumor response, 3 achieved partial response, with an overall response rate of 10.0% (95%
confidence interval(CI): 0.0-21.0%). Eleven patients (36.7%) showed stable disease and sixteen patients (53.3%) progressed
during the course of treatment. Median time to progression was 3.3 months (95% CI: 2.0-4.6 months), and median overall
survival time was 10.9 months (95% CI: 6.1-15.7 months) . Major grade 3/4 hematologic toxicity was neutropenia (17.6%) .
Three neutropenic fevers were observed during 131 cycles. The most common non-hematologic toxicities were grade 1/2
nausea/vomiting and diarrhea, which occurred in 13.8% and 2.7%, respectively. There was no treatment related death.
Conclusion
: The described modified FOLFIRI regimen appears a safe and effective salvage therapy in advanced gastric cancer
patients.
