- Haemophilus aphrophilus는 HACEK군에 속하는 구강정상 세균총으로 침습성 감염, 특히 감염심내막염을 주로 일으키고, 또한 골관절 감염을 일으킨다고 알려져 있다. 전 세계적으로 H. aphrophilus에 의한 감염심내막염 및 드물지만 골관절 감염에 대한 보고가 있었으나 H. aphrophilus에 의한 골관절 감염에 대한 국내 보고는 없었다. 저자들은 69세 여자에서 감염심내막염을 동반하지 않는 H. aphrophilus에 의한 흉추 부위 척추 골수염과 경막외 농양을 진단하여 항생제 투여와 수술적 처치를 통해 성공적으로 치료한 사례를 경험하였기에 문헌고찰과 함께 보고하고자 한다.

- Abstract
- Haemophilus aphrophilus is an aerobic, gram-negative oropharyngeal bacterium, commonly isolated from cases of HACEK endocarditis. In addition, H. aphrophilus has also been shown to cause invasive bone and joint infections. Although multiple cases of H. aphrophilus endocarditis have been described, no cases of invasive bone and joint infections caused by H. aphrophilus have been reported in Korea. Here we report the case of a 69-year old woman with a diagnosis of vertebral osteomyelitis and spinal epidural abscess with compressive myelopathy caused by H. aphrophilus, in which there was no objective evidence of infective endocarditis. She was successfully treated with intravenous administration of cefotaxime and drainage of the epidural abscess by laminectomy of the T3-7 vertebrae. (Korean J Med 2014;86:515-518)
